(New York) State Environmental Quality Review - Full Environmental Assessment Form or Short Environmental Assessment Form

Commonly referred to as "the State Environmental form"

Purpose
This form is used to assess the impact on the environment which a manufacturing project may have.

Note: This form must be signed and sent to the OCIDA office via postal mail for processing when completed (no faxes are allowed). Do not fill out this form unless you have completed OCIDA-SLAA. If you are requesting that OCIDA issue bonds on your behalf, this form MUST be followed by the completion of NP-IDAMP-BCQ.

Note: To fill out this form online, please go to www.dec.state.ny.us (under the category "Regulatory Information," click on "Impact Assessment (SEQR)", then read and follow the instructions on how to fill out the "Full Environmental Assessment Form" online). This form contains many questions. Even though many are comprised of simple check boxes, be advised that this form may take an hour or more to complete online. Applicants need only fill out the first ten (10) screens online.
